| 内容記述 | 本研究の目的は, 脳卒中後遺症をもつ患者のADL, コミュニケーションならびにこれらの低下による二次的問題に対する捉え方・解釈の仕方である知覚-情報処理と自尊感情との関連性を明らかにし, 加えて, 自尊感情に対する影響要因について検討することである. 研究方法は, 脳卒中後遺症によりADLが低下した65歳いじょうの在宅療養者155名に構成的面接調査と参加観察を行った. 分析方法は, χ2検定, t検定, 階層的重回帰分析を行った.結果として, 現在の自分のADL・コミュニケーション・対人関係・役割の各々について, 健康時と比較している群は, 最悪時と比較している群よりも自尊感情が明らかに低く, 捉え方の違いが自尊感情と関連性があることが判明した(p<0.05). また, 自尊感情へは, 役割の知覚-情報処理, ADLの客観的状態が明らかに影響を及ぼしており, さらにコミュニケーションの客観的状態に加え, これらの障害による二次的問題である役割や対人関係の変化に対する患者個々の捉え方, 解釈の仕方である内的プロセスに視点を当てることが重要であることが示唆された. | |||||||||||

| 内容記述 | A quantitative study was conducted to clarify the relevance of perceptual and information processing on self-esteem, and to determine factors influencing self-esteem. 128 patients aged 65 years or over, living at home with post-stroke single paralysis participated in the survey. Data were collected through a questionnaire and participant observations which addressed patients' ADL, communication capabilities, interpersonal relations, and roles. Analysis was conducted using Student t-tests, Chi-square tests, and hierarchical multiple regression. As a result, patients who compared their current health state with that prior to the onset showed significantly lower self-esteem than those who compared theirs with their initial highest distress (p<0.05). It was also found that the differences in patients' perceived roles and their objective status of ADL were related to their self-esteem,while their observed and perceived communication abilities were tended to be associated with their self-esteem. These findings suggest that to assist patients with post-stroke sequelae effectively, more attention should be paid to individual perceptions of post-stroke changes in interpersonal relations and roles. | |||||||||||
