研究目的は, 通所リハビリテーションに通う認知症の利用者が感動した体験を明らかにする. さらに, 感動前と感動体験時における五感への刺激, 利用者の表現(態度や感情)およびストレス指標に関する比較を行い, その差を明らかにすることである.対象は通所リハビリテーションに通う60代半ばの認知症の利用者1名である. 研究期間は平成17年9月から10月の4日間である. データ収集は, 参加観察で対象の行動観察を行うとともにストレス指標として唾液アミラーゼ活性値の測定を行った. データ分析は, 行動観察の内容は対象の行動や反応を経時的に文章化し, 感動の体験前と感動した体験に分けてまとめた. また五感への刺激, 対象の表現(態度や感情)および唾液アミラーゼ活性値について, 感動の体験前と感動の体験時を比較した. 倫理的配慮は, 研究の目的・方法について文書により対象に説明し, 対象が認知症で説明を十分理解できないため, 家族にも説明を行い文書で同意を得た. なお, 本研究は倫理委員会の承認を受けている.結果として, 感動の体験は, 対象が小学生と握手したときに生じた. その感動は, 研究者との接触やおやつを食べる間中持続した. 五感への刺激は, 握手という触覚であったが, その後は視覚, 聴覚, 味覚, 触覚において, 活発に表現がなされていた. さらに, 五感を通して, 思いと感情の交流がなされていた. 唾液アミラーゼ活性値はレクリエーション前と比べて+14kU/Lの40kU/Lであった. これらのことから, 認知症のある利用者に対するケアは, 五感への刺激や反応に注目して, 思いや感情が交流できる対応を行うことが大切であると考える. |

This study's aim is to clarify and examine the moving experience of the person with dementia who commutes to a day care.The subject was a woman in her middle sixties with dementia who commutes to a day care. The period of study was from September 2005 to October 2005. The observation of her behavior was always carried out. The measurement was conducted between the arrival at and the departure from the day care, and additionally before and after events in the day care. We itemized her actions and responses and compared her emotions in term of the stimulations to the five sense, her expressions and salivary amylase activities. The relation between the observation results and the difference of the salivary amylase activity before and after the events was analyzed. We eplained the aim and the method of this research by a document from the subject's family with mutual trust.The moving experience occurred when the subject shook hands with elementary school children. The moving was kept while touching with a researcher and eating snack. Asalivary amylase activity was 40kU/L. The stimulation to the five senses was started from shaking hands and expanded to sense of sight, hearing, taste and touch.It is suggested that the care noting of the stimulation to the five senses and responses is needed for the persons with dementia. |
