| 内容記述 | 本研究の目的は，訪問看護，訪問介護，居宅介護支援事業所従事者（以下，ケア専門職）が，在宅高齢者の終末期ケアにおいてチーム活動を行う際に経験する葛藤を彼らの語りから抽出し，葛藤の様相とその対処過程を明らかにすることである．研究対象は，熟練者によるよいチーム活動を行っている事例公募に応募した訪問看護，訪問介護，居宅介護支援事業所従事者である．在宅高齢者終末期支援5事例に関して対話的インタビューを行い，そのうち明らかに複数の葛藤が見いだせた3事例に対して現象学的方法を用いて分析した． 在宅高齢者終末期支援においてケア専門職が経験する葛藤は，「高齢者の意向と家族の意向との間で生じる葛藤」，「高齢者と家族の権力バランスの中で生じる葛藤」，「身につけている技量と，終末期ケアに要求される技量とのギャップから生じる葛藤」，「主治医の判断と家族の意向との間に生じる葛藤」，「生命の安全を守ることと家族の意向により介護行為を行うことへの葛藤」，「死に対するとらえ方に関する葛藤」など主に「間（はざま）」で生じていた．支援過程を分析した結果，家族が納得して看取ることを支援するという共通理念の存在，チームメンバー間の心理的・情緒的なサポートの存在が示された．また，ケア専門職は，看護，介護といった独自の専門性の中でケアの質の向上を追求する視点を持つと同時に，ケアを統合した場合にどのようになるのかという発想をもち，チームでケアの質を担保するという視点を有していた．専門職種としてチームメンバーをとらえるのではなく，高齢者ケアにともにかかわるチームという発想でチームメンバーをとらえ，葛藤を克服していた． |

| 内容記述 | The aim of this study was to use interviews to elicit the tensions experienced by professionals of home-visit nursing agencies, home-visit care agencies and care service planning agencies (referred to hereafter as care professionals) in implementing team activities in end-of-life care for home-based elderly people. Furthermore, this study aimed to identify aspects of the care professionals' tensions and processes of dealing with them. Participants were employees of home-visit nursing agencies, home-visit care agencies and care service planning agencies who responded to an advertisement for case studies of good team activities implemented by skilled professionals. A total of 5 cases of support for end-of-life care for home-based elderly people were identified, and interactive interviews were conducted for the 3 cases in which tension was evident. Interview data were analyzed using phenomenological methods. Tensions experienced by care professionals in support for end-of-life care for home-based elderly people were mainly concerned with the “between”, and included “tensions emerging between the wishes of the elderly person and the wishes of the family”, “tensions emerging in the balance of power between the elderly person and the family”, “tensions emerging from the gap between acquired skills and skills required for end-of-life care”, “tensions emerging between the judgment of the doctor in charge and the wishes of the family”, “tensions between protecting the life of the person and implementing care actions in accordance with the wishes of the family” and “tensions related to interpretations of death”. Results of the analysis of the process of care showed the existence of psychological and emotional support among care professional team members in team activities in the end-of-life care for home-based elderly people. Furthermore, care professionals were found to pursue improvement in quality of care within their own professional sphere of nursing or care, and simultaneously seek to secure the quality of care as a team while considering the consequences of integrating care. They overcame tensions not by seeing team members as a specific type of professional, but by conceiving team members to be a united team involved in the care of elderly people. |
